| Using Marketing Mayhem’s system, you are able to create powerful surveys to be completed by respondents online. |

Each survey is assigned a unique web address or URL, and respondents are requested to visit this URL in order to complete the survey. Alternatively, if you have the email address of each respondent (e.g. for an employee survey or customer survey), then you can send each one a personal invitation to complete the survey. The system can track the responses of each person identified, either by:
- requesting the respondent to enter some form of identification (such as an employee ID) before completing the survey, or
- creating a different URL for each respondent, and then tracking the individual responses.

| Not only can you specify the interaction with the respondent, but you are also able to customize the look of the survey to some extent. You can upload your own header and footer banner on the main landing page (which is the first page that the respondent sees when he opens your survey), and provide your own welcome instructions. You can also provide a logo which is visible throughout the rest of the survey. |

In addition to the standard value-added features of the Marketing Mayhem system, there are two features that are particularly useful for web-based surveys:
- the ability to display any web page to the respondent underneath the main question frame. This feature is particularly useful when the web page being displayed relates directly to the question being answered by the respondent
- the ability to display instructions or comments for the respondent, with reference to any question or building block that the respondent interacts with

| All respondent answers are captured immediately in real-time and stored on our server. If a respondent only completes part of the survey, then our system will recognize the survey as incomplete and mark it as such. If the respondent subsequently returns to complete the survey, the system will re-load the his or her partial responses. |
